Social media has become a powerful tool for businesses and organisations to promote their products and services. Non-profit organisations and government agencies are also exploring the potential of social media to reach their target audience.
One area where this could be particularly effective in increasing first responder (FR) engagement in case of cardiac arrest in the community. Influencers could be game-changer in this regard by partnering with non-profit and government agencies to create powerful campaigns that raise awareness about the importance of FRs and encourage people to take action. The systems saving lives ERC guidelines are clearly underlined and suggested to engage the community and raise awareness to improve survival.
The magnitude of local influencers lies in their ability to tap into the trust and loyalty of their followers, and they have already proved to be able to increase engagement, donations, and volunteer support for famous campaigns.
We recently published the factors associated with the arrival of smartphone-activated first responders before the emergency medical services in the Out-of-Hospital cardiac arrest in Emilia Romagna region with the use of a regional health app.
We described how, in 5 years with a small community of first responders in 13.4% of the case, they arrived before EMS and applied AED in 4%, delivering a shock in 0.9%.

However, only in 1 over five calls did at least one FR accept to intervene. To date, 14,520 over 3,923,045 people aged over 14 in Emilia Romagna subscribed to the DAE Responder project, representing 0.37% of potentially recruitable FRs.
This reflects a still underappreciated role of FR programs, which need to operate optimally and for which a social media campaign is cost-effective.
The clear and safe messages for the community should be:

Experienced operators at the EMS dispatch centre will guide you through chest compression procedures while waiting for the ambulance's arrival. They may also use video calling to improve the process;

Most of the FR apps commonly include a map of the AEDs available in the event of cardiac arrest to help you find them if you agree to intervene

In different parts of Europe, cardiopulmonary resuscitation and using AEDs from bystanders are protected by law in case of accidental harm to the victim

Sharing FR apps in Europe is a priority for community protection against OHCA; public awareness should include the creation of small proactive communities aimed at aggregating neighbourhood 'influencers' and communities of city and area 'first responders';
